Hexarelin is a synthetic growth hormone-releasing peptide (GHRP) that has been studied for its potential to stimulate the release of growth hormone (GH). This six-amino acid peptide has shown significant GH-releasing activity in both infant and adult rats, demonstrating its efficacy across different age groups [1].

Research has identified and characterized a new growth hormone–releasing peptide receptor in the heart, suggesting a direct influence of Hexarelin on cardiac function. A two-week pretreatment with Hexarelin showed beneficial effects, indicating its potential use in cardiovascular health [2]. Moreover, it has been found to induce penile erection and increase the concentration of nitric oxide in the paraventricular dialysate, further highlighting its diverse physiological impacts [3].

In humans, acute administration of Hexarelin has been observed to induce a short-lasting increase of cardiac contractility. This inotropic effect seems to be related to GH. Further studies have also revealed Hexarelin’s protective effects against ischemia-induced myocardial damage in aged and GH-deficient rats, suggesting its potential therapeutic benefits [4].

However, the transcellular permeation of Hexarelin remains a challenge, as its molecule is still too large for the epithelial cell membrane. This fact may partly explain why the use of permeation enhancers for oral peptide delivery has not evolved into a viable solution yet [5].

Hexarelin has been found to slightly increase PRL, ACTH, and cortisol levels in humans, besides strongly stimulating GH secretion. Interaction studies with somatostatin have also been conducted to understand more about the hormonal responses to Hexarelin. Lastly, research has indicated a selective lack of GH response to Hexarelin in patients with GH-releasing hormone receptor deficiency, adding another layer to our understanding of this peptide’s function [6].
